Provider Procedures

Terminating Services

There are two kinds of written agreements with the Guilford Center:
A contract
where the provider bills the Guilford Center directly for services provided under the contract.

A Memorandum of Agreement, which is non-financial because the provider bills the Division of Medical Assistance directly for services provided.

The Contract may be terminated:

  • At any time with the mutual written consent of both parties, executed by the Director of the Guilford Center and the provider.
  • By either the Director of the Guilford Center or the provider, upon 30 days written notice to the other party.
  • At any time for cause, without a time period, with written notice to the other party detailing the grounds for termination.

The Memorandum of Agreement may be terminated:

  • At any time with the mutual written consent of both parties, executed by the Director of the Guilford Center and the provider.
  • By either the Director of the Guilford Center or the provider, upon 30 days written notice to the other party.
  • At any time for cause, without a time period, with written notice to the other party detailing the grounds for termination.
  • The Memorandum shall be terminated immediately as such time as the provider is no longerdirectly enrolled.
