ARE YOU REQUIRED TO PAY A $3.00 CO-PAY AT THE GUILFORD CENTER?

The Guilford Center is required to collect a $3.00 co-pay at the time of service for some consumers who have Medicaid. However, you may not be required to pay this co-pay for some services.

We will inform you of your co-pay requirements during the fee-setting process or at the time of check-out.

IMPORTANT INFORMATION ABOUT MEDICARE PART D :
Anyone who has Medicare Part A and/or Part B, including people who have both Medicare and Medicaid, can join this federal program to help absorb the costs of prescription medication. The first phase of enrollment for Medicare Part D began in November 2005 and ended May 15, 2006. However, a new enrollment period starts in November 2006.
